ENROLMENT – A whole term of classes, a uniform and Annual Licence. Please complete and submit the below form, then click “add to basket” to purchase the enrolment.

After School Club Enrolment Form
Used to enrolment new members into the After School Club. This will then make you a member of Phoenix School of Martial Arts and Cobra Martial Arts Association.
I the trainee indemnify the Cobra Martial Arts Association and/or Phoenix SMA in respect of any injury caused to another member or against losses caused to the above mentioned by any act of mine or by default. I the trainee understand that the training and practice of the martial arts can be dangerous, and I agree personally to bear all losses caused by injury whilst engaged in it.

Your enrolment includes a whole term of classes (at least 10), a uniform and a years licence and membership.
After the enrolment period, you must purchase the next half term or full term to maintain your place in the After School Club.
